PROGRAM OBJECTIVES

Day Training / Training Camp: I understand that my dog’s training program is individually tailored to meet the goals and objectives agreed upon by my dog’s personal trainer and me.

Personal Training: I understand that my dog’s physical fitness program is individually tailored to meet the goals and objectives agreed upon by my dog’s personal Fitness Coach and me.

Group Training: I understand that any group training program is designed to accommodate multiple dogs with varying goals and fitness levels.

Personal in Home Training: I understand that my dog’s in-home training program is individually tailored to meet the goals and objectives agreed upon by my dog’s personal trainer and me.

TWO PAWS UP DOG TRAINING, INC., POLICIES

Our classes are taught in a clean and dog friendly environment with a focus on positive reinforcement-based force-free training. Therefore, the use of punishment and aversive equipment, such as choke, prong, pinch, or electronic collars (whether they provide vibration or shock), is not permitted in any of our classes. We also do not allow extendable leashes, even when locked, for the safety of our handler/canine teams.

DESCRIPTION OF THE EXERCISE/CONDITIONING PROGRAM

I understand that my dog’s training and/or exercise program will involve participation in a number of types of activities.These activities will vary depending upon the objectives that my dog’s personal trainer and I establish. Activities may include:

  • Aerobic activities including, but not limited to, the use of treadmills;

  • Muscular endurance and strength building exercises including, but not limited to, the use of weights, resistance bands, calisthenics, and exercise equipment;

  • Other activities selected by my canine personal trainer and agreed upon by me; and

  • Selected physical fitness and body composition assessments.

DESCRIPTION OF POTENTIAL BENEFITS

I understand that regular training, canine fitness and conditioning programs have been shown to have definite benefits for Canine Total Health and Well-Being. I know that some of the benefits can include better quality of life at home, weight reduction (if needed), reduction of body fat, improvement of cardiovascular function, improved strength and muscular endurance, improved posture, speed, quickness, proprioception, power, balance and flexibility.

PARTICIPANT RESPONSIBILITIES

I understand that it is my responsibility to:

  • Fully disclose any of my dog’s health issues or medications that are relevant to participation in a training and/or strenuous exercise program;

  • Have my dog cease exercise and report promptly any unusual feelings (e.g., difficulty breathing, apparent injury, lameness, refusal to eat or work) during the exercise program/training class;

  • Clear my dog’s participation in any canine fitness activity with my veterinarian.

PARTICIPANT ACKNOWLEDGEMENTS

In agreeing to this canine personal fitness program:

  • I acknowledge that my dog’s participation is by my decision

  • I understand the potential physical risks involved in participating in a training and/or exercise program and believe that the potential benefits outweigh those risks.

  • I give consent to certain physical touching that may be necessary to ensure proper technique and body posture.

  • I understand that the achievement of behavioral, health and/or fitness goals cannot be guaranteed.

  • I have had a voice in planning and approving the activities selected for my dog’s training/exercise conditioning program.

  • I have been able to ask questions regarding any concerns I might have, and have had those questions answered to my satisfaction.

  • My dog is in good and healthy physical condition, has no impairment which might prevent participation in such activities, and I have been advised to consult with a veterinarian prior to beginning this program.

  • I have been advised to have my dog cease activity immediately if I see that my dog is experiencing unusual discomfort and or if I feel the need to have my dog stop the program.

I have read and understand the above agreement; I have been able to ask questions regarding any concerns I might have; I have had those questions answered to my satisfaction; and I am freely acknowledging this agreement.

TERMS AND CONDITIONS

Listed below are our terms and conditions that you accept when you register for one of our classes, private lessons, or day training.

WAIVER, ASSUMPTION OF RISK, & AGREEMENT TO HOLD HARMLESS

I understand that no training or exercise program is without inherent risks regardless of the care taken by a canine personal trainer and that my dog’s personal safety cannot be guaranteed. I realize that when my dog is participating in any exercises, particularly those that induce cardiovascular stress, there is a slight chance of serious injury (e.g. cardiovascular accident) or catastrophic incident (e.g., death, paralysis). Likewise, I know that engaging in muscular endurance, strength building, and other fitness activities sometimes results in minor injuries (e.g., bruises, musculoskeletal strains and sprains), or less frequently, more serious injuries (e.g., muscle tears, herniated disks), and rarely, catastrophic injury (e.g., death, paralysis).

I understand that attendance at a dog training class or private lesson is not without risk to myself, members of my family or any of my guests who may attend, or to my dog. Some of the dogs that I (we) will be exposed to may be difficult to control and may be the cause of injury even when handled with the greatest of care. In consideration of, and as an inducement to, the acceptance of my registration for classes, whether online, over the phone, or in person, I agree to indemnify and hold harmless and hereby waive and release Two Paws Up Dog Training, Inc., its employees, owners, and agents from any and all liability of any nature of any damage or injury which I, my family, guests, or my dog might suffer. I expressly assume the risk of any such damage or injury while attending any training sessions or other dog related functions while at Two Paws Up Dog Training, Inc., location or any location a class is held, or at any location a private lesson may be held. I also understand that the degree to which a dog is successfully trained is a function of the interest, commitment, and cooperation of the owner. I acknowledge and agree that there is no guarantee that my dog will achieve the desired level of training, despite the best efforts of the instructor or instructors.

CANCELLATION POLICIES

Due to the high level of service we offer our clients, and the nature of our business, we have the following cancellation policies. We strive to offer our clients small, safe, well-staffed classes and other training opportunities.

CANCELLATION POLICY FOR CLASSES

For our OPEN ENROLLMENT CLASSES (all puppy classes) you have eight consecutive weeks to finish your six sessions of class. There are no “make up classes” for a specific lesson, however you are welcome to attend a different session with prior permission from the instructor of the class you wish to attend. We do not guarantee that if you attend the same class type at a different day or time that you will not repeat a session you have already attended.

  • After registering for a class, if you need to cancel your start date and do so more than one week before the start date, you will receive a 100% refund.

  • After registering for a class, if you need to change your start date, you may change it one time without penalty if requested 72 hours (3 days) prior to your start date.

  • If you cancel your registration for class in less than 72 hours prior to your chosen start date, you will receive an 80% refund.

  • Once you have started attending class there are no refunds.

For our GROUP CLASSES that are offered on a set schedule (New Additions, Tricks, Nose Work, or any other specialty classes) there are no makeup classes.

  • A 100% refund will be given if you cancel a specialty group class seven days prior to start.

  • An 80% refund will be given if you have not yet started class and it is less than 1 week before the start of the class.

  • No refunds are available if cancellation occurs less than 1 week before the start of class.

  • Once you have started attending class there are no refunds.

The following policies are effective for our MEMBERSHIP PACKAGES:

  • If you purchase a membership package, do not attend classes that are included within that package, and cancel within one week of your purchase date, you will be refunded 80% of your package price.

  • If you purchase a membership package, attend any class included within that package, no refunds will be given.

  • There are no extensions to packages past their expiration date. We are also unable to temporarily halt a package membership midterm.

The following policies are effective for PAID WORKSHOPS AND SEMINARS:

  • Cancellations 30 days prior to a paid workshop or seminar are given a 90% refund.

  • Cancellations 2 weeks prior to a paid workshop or seminar are given a 50% refund.

  • No refunds for cancellations less than 1 week prior to the workshop.

CANCELLATION POLICIES FOR PRIVATE LESSONS

Private lessons must be cancelled at least 48 hours prior to appointment. Cancellation that occurs in less than 48 hours’ time will be subject to a 50% cancellation charge. No shows will be charged the full fee of the applicable lesson. If the lesson was part of a package, the lesson will be counted as one full lesson.

  • You may reschedule a private lesson—please give up to 72 hours (3 days) prior to your lesson when possible.

  • No refunds will be given if cancelled less than 24 hours prior to your scheduled lesson.

  • No refunds for unused portions of lesson packages.

  • No extensions to lesson packages past their expiration date.

CANCELLATION POLICIES FOR DAY TRAINING

  • After registering for Day Training, if you need to cancel or make changes to your regular day you must do so more than 1 week before the start date. If you cancel you will receive a 90% refund one time without penalty.

  • If the dog is not present at their scheduled appointment, the day will be counted as a completed session.

  • Excessive cancellations or no shows may result in dismissal from the program, at the trainer’s discretion.

MEDIA RELEASE99

I hereby consent to use of my and/or my dog’s photography, videography, likeness or voice on the Internet, or on any other electronic/digital/print media. I agree to release and hold harmless Two Paws Up Dog Training, Inc., FitPAWS® (Ball Dynamics International, LLC), Do More With Your Dog, National Association of Treibball Enthusiasts, and the American Treibball Association from and against any and all claims, demands, actions, complaints, suits or other forms of liability that shall arise out of or by reason of, or be caused by such use of my or my dog’s photograph, likeness or voice. It is further understood and I do agree that no monies or other consideration in any form, inducing reimbursement for any expenses incurred by me, will become due to me, my heirs, agents, or assigns at any time because of my and/or my dog’s participation in any of the above activities or the above-described use of my or my dog’s photography, videography, likeness or voice.
